Pharmacy Department Overview

The DMC Sinai-Grace Department of Pharmacy is committed to bringing together pharmacy practitioners whose clinical expertise, research and educational backgrounds are utilized to advance a patient-oriented practice through the application of pharmaceutical care and education of patients, physicians, nurses and other health care providers. The department is committed to providing pharmaceutical care through the safe, effective, rational, ethical and cost effective use of medications in our institution. As a concerned patient advocate, the DMC Sinai-Grace pharmacist shares in the ultimate responsibility of patient outcomes.

Operations

The pharmacy operates a 24-hour computerized unit dose drug distribution system with a comprehensive IV admixture service. The department uses the latest technology, such as Pyxis Profile, an integrated hospital-wide electronic medical record with prescriber order entry and bar code medication administration. All of this technology allows the departmental staff to provide a pharmaceutical environment that will reduce errors and streamline the medication process.

Clinical Services

Our clinical pharmacy services illustrate the unique blend of professional challenge, commitment and concern that makes pharmacy an important part of the healthcare team. Clinical pharmacists are decentralized throughout the institution and work integrally with the medical team. Services include, but are not limited to:

  • Inpatient pharmacokinetic and anticoagulation dosing
  • Renal dose adjustment
  • Attendance at code blues
  • Criteria drug monitoring
  • Participation in patient care rounds

Services are provided to all medical/surgical units, rehab, psych and emergency departments. Additionally, clinical pharmacists specializing in neonatology, infectious disease, critical care and OR services, working closely with our IV therapy nurses, provide added expertise to support the department’s mission and goals.

Ambulatory Services

Pharmacy-managed ambulatory services are provided in various medical clinics, including anticoagulation, congestive heart failure, infectious disease/HIV and general medicine. In addition, support is provided to the Cardiac Rehab and Diabetic Connection Programs.

An on-site Outpatient Pharmacy is available to fill prescriptions to the community, discharged patients, employees and patients from outpatient clinics. The pharmacy has dedicated pharmacists and technicians to answer all your medication questions. We accept most insurance plans.
